  The informant on Roy's death certificate was William Stafford, Jr. The un dertaker was Fred Dering, Morgantown, WV. In her Descendants of James Stafford http://familytreemaker.genealogy.com/users/f/o/r/Caroline-Stafford-for bes/ ODT1-0001.html Caroline Stafford Forbes shows Roy as John Lee "Roy" Staf ford. While this is probably accurate, I do not have information to confi rm it, and am using the name Roy, since that is the only name shown for h im in censuses and on his death certificate.
